Contents would could include:<\/p>\n<div align=\"left\">\n<ul type=\"disc\">\n<li>Information concerning immunizations (with the date)<\/li>\n<li>List all medications your dog is receiving or has received<\/li>\n<li>Any parasite or flea medication which your dog is taking should also be noted<\/li>\n<li>Records of any blood tests and results<\/li>\n<li>Copies of veterinary invoices and exam sheets.
